If you are a staff member,

  • and this is your first time serving (or first time serving in 2012) please NOTE: There will be a NO new form for 2012. Please fill out the information form at workshop or ahead of time at weekly prayer meetings.

  • ALL WAIVER / RELEASE FORMS and DONATIONS are due BEFORE or AT WORKSHOP. If it isn't turned in, a $ 5 adminstrative fee will be added.
